How Does Getting a Comprehensive Medication Review Work?

First, please review this website in full, so you understand what this service is providing. To review, this is a comprehensive medication review, specifically for those 65 years and older. I will not be prescribing any medications for you. You will receive a detailed medication review with recommendations and considerations to discuss with your primary care doctor who knows you best. My contact information will be on the recommendations for communication with your PCP if they would like to discuss.

After you book the consult and complete payment, you will be sent a detailed medical history intake form that will need to be completed by you, with help from your caregivers if needed. It will also ask you what your current concerns are, along with completion of full medication list including all supplements and over the counter products (because all over the counter medications and supplements are NOT safe for you).

Once you complete and submit the intake form, it will be sent back to me. I will do a detailed, comprehensive assessment. I may request a phone call, if I need more information, or if it is complex case. Once I have it completed, I will send it back to you. It will include recommendations, and considerations for you to print, or take to your next appointment with your primary care provider. In this assessment, recommendations may be made to stop medications, reduce doses of specific medications, substitute medications for a safer alternative, or any combination of the above.
