A Comprehensive Geriatric Medication Review is a deep-dive into your medications. This will be performed by an expert Geriatric Nurse Practitioner. 
Your primary care provider isn't given the time to do this in our current healthcare model. Often times, they don't have the geriatric specialization to take on this complex task. I have become very passionate about medication management in the elderly and it's numerous negative outcomes. 

Each review will provide:
- Identification of high risk medications for adverse    side effects.
- Reduction of unnecessary medications.
- Identify medications that should be avoided in          those over the age of 65.
- Identify potential drug-drug interactions that            exist with your medications.
- Make recommendations for a safer alternative          medication when one exists.
- If you currently report possible side effect                  symptoms, I will identify which are the most likely    cause.
- Make recommendations for any changes in your      medications that may optimize your function            improve quality of life.

Polypharmacy, or taking many medications at the same time, is a very common problem in our seniors. 
- 50% increased risk of falls, or hospitalization.
- 55% of the elderly report medication side effects.
- 700,000 emergency room visits related to                  polypharmacy, or medication mismanagement          yearly.
- Approximately $1.7 billion in avoidable ER                visits and hospitalizations annually.

Common adverse side effects of medications include confusion, falls, weakness, loss of appetite, dizziness, fatigue, and nausea just to name a few. I will perform a comprehensive medication review, taking into account your current symptoms and make recommendations, so you can become educated and share with your primary care provider for discussion at your next appointment!
